information for new clients

Preparing for your first visit

As a Licensed Clinical Social Worker, I offer services to individuals, children, adolescents, couples and families. I collaborate with my clients to improve emotional, behavioral and relationship issues and strengthen connections with family, friends and community.

In the first session we will work together to evaluate your needs and come up with a plan that is right for you. If you have specific goals in mind, please let me know. I provide an atmosphere of respect and understanding.

Fees for evaluation and treatment services are based upon the session length, and type of service provided. In addition to regularly scheduled appointments, fees are charged for other services requiring my professional time and expertise. These services may include consultations with teachers and school officials, preparing written reports and telephone consultations.

I accept insurance from most companies. The best way to find out if your insurance will pay for services is to look on the insurance company's website or call them and ask about what your plan specifically covers. You will need to give them my name and credentials when you call.

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental health benefits?
  • Do I have benefits through my employer (EAP – Employee Assistance Program)?
  • What is my deductible and has it been met?
  • How many sessions per calendar year does my plan cover?
  • How much does my plan cover for an out-of-network provider?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?
  • What is my co-pay?

Please bring a form of payment when you come for your visit. Payment can be in the form of cash or check.

Cancellation Policy

If you need to cancel an appointment, I require notification at least 24 hours in advance or you will be billed directly for the full cost of your missed session. When you schedule an appointment, I make a commitment to see you during that time, and you commit to pay for the time that I have reserved especially for you. If you miss more than three appointments in a row, or if canceling on short notice becomes habitual, I may choose to terminate our treatment relationship.
